CHILD PASSPORT CHECKLIST

* If your child has yet to receive a Social Security #, you must show documented paperwork that one has been requested.
* Complete and print 2 copies (Single-Sided Only) of the Online Government Passport Application Form DS-11.
* Hand-written applications are NOT accepted.

* Color photo must be 2 x 2 inches and have a white background and be professionally taken within the past 6 months.
* Eyeglasses are no longer allowed to be worn in passport photos.
* We recommend using the Passport Photo Creator mobile app. You will then pick up your photos at your nearest Walgreens location.

* Must be an ORIGINAL or CERTIFIED copy with a raised or embossed seal of an U.S. Birth Certificate (with both parents names on it).
* An Original Naturalization Certificate is acceptable.
* A Marriage Certificate or Divorce Decree is necessary if a parent's last name has changed.
* If the minor child has a current/recently expired passport, it must be surrendered with the order.

* A valid copy of ID that has been issued GREATER than 6 months ago must be included (Front & Back Copy) for each parent.
* ID can be a Drivers License, State ID, Military ID, or any Gov't ID. Paper, Learner's Permits and temporary licenses are not acceptable forms of ID.

* Check or money order made payable to US Department of State.
* No starter or temporary check can be used.
* The applicant's full name and date of birth must be written in the memo section.
* Child 15 years or younger fees are $160 (Passport Only) or $175 (Passport and Card).
* Child 16 or 17 years old fees are $190 (Passport Only) or $220 (Passport and Card).

* Most commonly used itineraries are airline, cruise & hotel reservations. The Travel Itinerary must match the application and include the child's name.

* If one parent/guardian cannot attend the Passport Acceptance Agency visit, the following form must be complete and notarized.

* This Letter gives RushMyPassport permission to represent your child at the U.S. Department of State.
* Ensure that the Courier Company Name is printed as it appears at the top of that page. This will help avoid processing delays. Hand print and sign in BLACK INK ONLY.

DO NOT VISIT THE PASSPORT ACCEPTANCE AGENT BEFORE CALLING OUR OFFICE FOR THE LETTER OF AUTHORIZATION.
* The documents will be witnessed, signed & sealed at a Courthouse or local Post Office for a fee of $35.00.

* The Pre-paid Overnight Shipping Label is included in the Step 2 email, along with the Letter of Authorization form.
* Print this label and place on the outside of an overnight shipping envelope.
